Welcome to Stellar Lab & Training Center. These Terms and Conditions govern your use of our services, including CPR certification, Nurse Aide competency testing, genetic testing, drug screening, and background checks. By accessing our facility or using our services, you agree to comply with and be bound by the following legal provisions designed to ensure technical precision and safety in healthcare standards.
Service Usage Rules
01. Authorized Testing & Certification
All CPR certification and Nurse Aide competency testing must be scheduled in advance. Participants must present valid government-issued identification to be eligible for testing and state registry alignment.
02. Specimen Collection Protocols
Specimen collection for DNA, genetic testing, and drug screening must follow strict chain-of-custody procedures. Mobile services are subject to environmental suitability checks to ensure the integrity of the collected samples.
03. Professional Conduct Standards
All clients and students are expected to maintain professional conduct within the training center. Interference with testing equipment or disruptive behavior during skills competency exams will result in automatic disqualification.
04. Privacy and Confidentiality
Stellar Lab & Training Center complies with healthcare privacy standards. Information obtained during background checks, drug screenings, or competency testing is handled with strict confidentiality and shared only with authorized parties.
Payment & Billing
All service fees for CPR certification, nurse aide testing, and lab collections must be paid in full at the time of scheduling or upon arrival at our facility, unless corporate billing arrangements have been previously established. We accept major credit cards, technical nursing school vouchers, and authorized employer payments.
Cancellations for skills testing must be made at least 48 hours in advance to be eligible for rescheduling. Missed appointments without prior notice are non-refundable due to the specialized nature of our technical medical evaluations and administrative registry filings.
Limitation of Liability
Accuracy of Results
Stellar Lab & Training Center provides testing and certification services with the highest degree of professional care. However, we do not guarantee the absolute accuracy of laboratory results provided by third-party processors or the final employment outcomes following nurse aide competency testing. Our liability is limited to the cost of the specific service rendered.
Service Scope
Under no circumstances shall the center be liable for indirect, incidental, or consequential damages arising from the use of our testing facilities, mobile collection services, or certification training programs, including but not limited to loss of wages or clinical placement delays.
Training and Certification
While our CPR and nurse aide competency programs are state-approved, certification is granted upon successful completion of required performance standards. Stellar Lab & Training Center is not responsible for the performance or actions of individuals once they have obtained certification through our programs.
Indemnification
Users agree to indemnify and hold harmless Stellar Lab & Training Center from any claims, damages, or legal fees resulting from false information provided during intake or misuse of certification credentials by the user.
